Dental Membership Program Guidelines

Our dental membership plan is not insurance. The program is offered to individuals and families who do not have insurance, or who choose not to utilize their insurance. Dental membership program benefits are not transferable to another dental practice or dental specialty practice. Payment is always due at the time of service.

  • Membership is for 1 year, beginning on the enrollment date.
  • Membership dues are payable in full upon enrollment and are non-refundable.
  • Cash, check, or credit card payments are accepted.
  • Membership will automatically renew on the anniversary date unless a written request to cancel is received 30 days prior to the renewal.
  • Payments for additional dental services are the member’s responsibility. Payment is due on the date of service to qualify for the discounts detailed above.
  • Membership must be current to receive the discount.
  • Fees for dental services may change at any time.
  • It is the member’s responsibility to schedule and keep all appointments offered as part of the Dental Membership Program.
  • Please notify our office at least 48 hours in advance if you must change a scheduled appointment.
  • There are no deductibles, no pre-authorization, no yearly maximums, and no waiting period.
  • There are no exclusions for elective care.

How do I sign up for the Membership Program?

Fill out and submit the form, and a Shawsheen Family Dental representative will contact you to answer any questions you may have and to get you started with the program. We understand that dental discount plans vary between providers, so please don’t hesitate to contact us with questions.

Do you have other dental discount plans?

Our Dental Membership Program allows you to receive the preventive care you need to have healthy teeth and gums. While we do offer cash discounts and have financing available through Wells Fargo, this dental discount plan is slightly different. Please call our offices if your questions are not answered after reviewing this page.

Is this an insurance plan?

No, this program is not an insurance plan. Our Dental Membership Program provides our patients with discounted prices on preventive care, as well as a range of common dental procedures. You will pay the discounted rate at the time services are provided.

Can I sign up my whole family?

Yes, you can sign up an entire family or any individual. This offer is not valid when using dental insurance of any kind.

I am an existing patient. Can I join?

All existing patients are invited to join our Dental Membership Program.

What happens if I become insured while a member of the program?

Our Dental Membership Program is for uninsured patients or patients who choose not to use their insurance (for example, if they have out-of-network insurance). If you become insured or choose to use your insurance, your membership will be terminated. Dues are non-refundable.

I have dental insurance. Can I sign up for this Membership Program?

This offer is not valid in combination with dental insurance of any kind. We sincerely appreciate your loyalty to our practice.

I have a FLEX plan. Can I submit this annual fee for reimbursement?

Most FLEX plans reimburse dental procedures, but we advise you to contact your employee benefits provider to confirm the parameters of their coverage.
